Dear Dr. Lee,
Mr. Ding Thong presented at my clinic today with a pain in the right angle of the lower jaw. Examination and radiographs reveal that he has deep periodontal pockets in 46 and 47 as well as horizontal impaction in 48. He requires surgical extraction of 48 as well as treatment for 46 and 47.
On initial examination more than a year ago, we found that his oral hygiene was very poor. He also needed replacement of his co/cr denture in relation to 11, 21 and 24.
Instruction on oral hygiene was given and his oral health has improved considerably. All the teeth were scaled and cleaned. The patient’s dentures have also been replaced.
We would be grateful if you could treat 46&47 for pockets and extract 48 surgically. Recent radiology has revealed no abnormalities and good levels of bone support. I have enclosed today’s radiograph.
Please note that the patient is Hepatitis B –ve.
