WRITING TASK:– DENTISTS

"Using the information in the case notes, write a letter of referral to the oral surgeon to remove 48 surgically & treat 46 & 47 for pockets. Address the letter to Dr. Alex Lee, 12 Prendergast Ave, Marine Bay, Hong Kong-3032."

Do not use note form in the letter, expand the relevant case notes into full sentences. The letter should be 15-20 lines long. Only the first 25 lines will be assessed.

Reason for Presenting	Patient complains of pain in the right angle of mandible. Dull pain since 10 months. Pain is severe since a week

Medical History	- War injuries - Hepatitis B-ve
Family & Social History	- Worked in Armed Forces - Participated in war - Native of Vietnam - Living in Australia since 3 years
Initial Examinatio	- 10/07/07 - All teeth present except 11, 21 & 24 - Oral hygiene poor - Occlusal Amalgam 26& 36 - Caries 14, 46, 45 & 35

21.07.07	MOD Amalgam 26&36
21.08.07	Oral hygiene much improved. Scaled and cleaned. 25.09.07
25.09.07	Replacement of dentures in 11, 21 and 24
26.10.08	Patient came with pain in the right angle of the lower jaw. Radiograph showed deep periodontal pockets in 46 & 47. Horizontal impaction in 48. Removal of 48 surgically and treat 46 & 47 for pockets.
